10 Problems in Cardiovascular Care
and How We're Solving Them
Reactive Care
Today's healthcare system is largely designed to monitor, manage, and medicate disease after it develops, not detect and prevent it in its earliest stages. As a result, valuable opportunities for early intervention are often missed.
Too often, people are told they're "fine" until:
- Symptoms appear
- Lab values cross disease thresholds
- Imaging reveals advanced disease
- A heart attack, stroke, or other cardiovascular event occurs
We don't watch and wait. We act early.
Proactive Care
We believe the future of healthcare is proactive, personalized, and prevention-focused.
That's why we encourage comprehensive cardiovascular screening as early as your 30s and 40s, including:
- Advanced cardiovascular biomarkers
- AI-assisted coronary CT angiography, which can show plaque type and volume
These tools detect the hidden disease processes that conventional screening often misses — when prevention, stabilization, and even regression are still possible.
Earlier detection allows you to:
- Identify disease before symptoms develop
- Address the root causes of cardiovascular risk
- Make targeted lifestyle changes
- Track progress and adjust treatment strategies over time
- Change the trajectory of your health
Missing High Risk Patients
Conventional cardiovascular screening frequently misses people with significant heart disease, creating false reassurance.
Normal cholesterol, blood pressure, body weight, or even a normal stress test or calcium score do not guarantee a low risk of heart disease. Because heart disease develops silently over decades, many people have advanced plaque long before symptoms or traditional tests become abnormal.
In fact:
- More than 50% of people who have a heart attack had no prior symptoms
- 50% have normal cholesterol
- 25% have a coronary calcium score of zero
- 70% pass a stress test shortly before their heart attack
- Landmark studies (Framingham, ARIC, and MESA) show that 60–80% of heart attacks occur in people not classified as high risk
Test, Don't Guess
We believe better care starts with better information. You are the CEO of your health, and you deserve to be educated, empowered, and equipped with the information possible to make informed decisions about your health.
Our comprehensive approach combines:
- Advanced cardiovascular biomarkers to identify inflammation, metabolic dysfunction, cholesterol particle abnormalities, and other hidden drivers of disease
- AI-assisted coronary CT angiography (CCTA), a scan that can show plaque, plaque burden, and high-risk plaque characteristics
- Whole-person cardiovascular assessment that evaluates the interconnected factors influencing cardiovascular health, including metabolic health, lifestyle, nutrition, sleep, stress, hormones, environmental exposures, and other root contributors.
Oversimplified Risk Assessment
Conventional cardiovascular care relies heavily on oversimplified risk calculators that estimate risk using a small number of variables, including age, sex, total and HDL cholesterol, blood pressure and diabetes status, and smoking.
While useful to an extent, these calculators were designed primarily to guide medication decisions, not to explain why someone is at risk. As a result, they often miss important biological drivers of heart disease and falsely reassure people with significant underlying disease.
It's common for two people with the same calculated risk score to have dramatically different actual cardiovascular risk.
Evaluate the Whole Person
We look beyond a risk score to understand the complete picture of your cardiovascular health. By combining advanced labs, AI-assisted imaging, and a comprehensive global health assessment, we identify the factors driving your individual risk — helping prevent underdiagnosis, overtreatment, and treatment focused on the wrong problem.
We evaluate key contributors across multiple areas, including:
- Cholesterol & lipoproteins: particle number, particle quality, and other advanced lipid markers
- Metabolic health: insulin resistance, diabetes, blood pressure, body composition, fatty liver disease, PCOS, gout
- Inflammation & vascular health: chronic inflammation, oxidative stress, endothelial dysfunction, autoimmune disease, periodontal disease
- Nutrition & gut health: diet quality, vitamin and mineral status, digestion, microbiome health, nutrient absorption, gallbladder and liver function
- Lifestyle: exercise, sleep, stress, emotional health, relationships, alcohol, smoking, caffeine, recreational drugs, and occupational factors
- Environmental exposures: mold, heavy metals, toxins, plastics, pesticides, air and water quality, occupational exposures
- Infections: tick-borne illness, chronic viral infections, microbiome imbalances, and parasites
- Other medical factors: hormonal health (including menopause and andropause), thyroid and kidney disease, chronic pain, medications, and family history
Imaging That Misses Dangerous Disease
Coronary artery calcium (CAC) scoring is the most commonly ordered cardiovascular imaging test — if imaging is ordered at all. While CAC detects calcified plaque, it cannot identify the earliest and often most dangerous forms of coronary artery disease, including:
- Non-calcified plaque
- Lipid-rich plaque
- High-risk (vulnerable) plaque
- Early atherosclerosis before calcification develops
A calcium score of zero does not guarantee healthy arteries. Approximately 20–25% of acute coronary events occur in people with a CAC score of zero, largely because many heart attacks result from rupture of non-calcified, vulnerable plaque, not stable calcified plaque.
The Complete Picture
When clinically appropriate, we help you get an AI-assisted coronary CT angiography (CCTA), a scan that directly shows your coronary arteries.
Unlike a calcium score, a CCTA can show:
- Total plaque burden
- Calcified and non-calcified plaque
- Lipid-rich, high-risk plaque
- Plaque composition and morphology
- Arterial narrowing (stenosis)
- Early atherosclerosis before calcification develops
Instead of a single calcium score, you receive a comprehensive map of your coronary arteries, showing where plaque is located, how much is present, what type it is, and whether it has high-risk features associated with future heart attack.
"Normal" Doesn't Always Mean Healthy
Most laboratory results are interpreted using population-based reference ranges, which reflect what is statistically common, not what is optimal for health.
As a result, many people are told their labs are "normal" while early metabolic dysfunction, inflammation, nutrient deficiencies, and cardiovascular risk are already developing.
Insulin resistance is a prime example. It often exists for years before blood sugar or HbA1c rises enough to meet the criteria for prediabetes or diabetes, allowing vascular damage and metabolic dysfunction to progress unaddressed.
Optimal Range Analysis
We interpret biomarkers using functional optimal ranges to identify early metabolic dysfunction and biological stress, creating opportunities for prevention long before disease develops.
Rather than simply labeling results as "normal" or "abnormal," we show where you fall on the continuum from optimal health to disease.
For example, with insulin resistance, we begin identifying risk and recommending intervention when HbA1c reaches 5.5–5.7%, while conventional care often waits until 5.7% (prediabetes), 6.0%, or even 6.5% (diabetes). We also measure fasting insulin — a marker that is often elevated for years before blood glucose or HbA1c becomes abnormal, yet is rarely checked in routine care.
Limited Access to Advanced Testing
Many people seeking a more complete understanding of their cardiovascular health cannot access advanced testing, including:
- Advanced lipid, metabolic, inflammatory and full body system biomarkers
- Coronary CT angiography (CCTA)
This happens for two main reasons:
- Many clinicians do not routinely order these tests because they fall outside traditional screening guidelines.
- Insurance companies often deny coverage when testing does not meet their medical necessity criteria.
Testing Without Barriers
We make advanced cardiovascular assessment more accessible by combining advanced laboratory testing with precision cardiovascular imaging when appropriate.
Through our direct-to-consumer platform, testing is guided by your individual risk profile and health goals, not limited by traditional screening pathways or insurance restrictions.
The result is a more complete understanding of your cardiovascular health, helping uncover hidden risk, generate actionable insights, and support better-informed conversations with your healthcare team.
Medication-Centered Care
If all you have is a hammer, everything looks like a nail.
Conventional cardiovascular care often focuses heavily on medications and lab values while overlooking the underlying biology driving disease.
Common limitations include:
- Treating numbers instead of biology and root causes
- False reassurance from improved cholesterol values
- Limited attention to metabolic health, inflammation, lifestyle, and environmental factors
While medications like statins can be important and lifesaving for many people, they do not directly address many of the causes of elevated cholesterol and coronary artery disease:
- Insulin resistance
- Chronic inflammation
- Thyroid disease
- Vitamin and nutrient deficiency
- Poor nutrition
- Sedentary lifestyle
- Poor sleep
- Environmental exposures
Treat the Why, not the What
We don't just ask, "What medication do you need?" We ask, "Why did this disease develop in the first place?" That question guides our approach.
Heart disease is rarely caused by a single abnormal lab value. It results from the complex interaction of genetics, metabolism, inflammation, nutrition, sleep, stress, fitness, hormones, oral health, gut health, environmental exposures, and other factors.
Medications can be an important tool, but they are only one piece of the puzzle. Improving a cholesterol number does not necessarily mean the underlying drivers of disease have been addressed. Both patients and clinicians can be falsely reassured when a medication improves a lab result while root causes remain unchanged.
Our goal is not simply to improve a number — it is to improve the biological processes that drive cardiovascular health. Because most cardiovascular risk is linked to modifiable factors, lifestyle remains one of the most powerful tools for prevention.
Misunderstanding Cholesterol
Cholesterol is one of the most misunderstood topics in cardiovascular healthcare. Many people, including well-intentioned clinicians, view and treat it too simplistically.
The reality is more nuanced:
- High cholesterol is not always harmful.
- Low cholesterol is not always protective.
- Lowering cholesterol numbers does not automatically translate into reduced risk
The same cholesterol number can mean very different things in different people. Some individuals with low cholesterol have significant coronary plaque, while others with high cholesterol have little or no plaque burden.
It would be easier if high = bad and low = good, but cardiovascular biology is far more complex. A complete assessment of what's driving cholesterol levels and cardiac disease requires looking beyond a standard lipid panel and considering:
- LDL particle number, size, and density
- Lipoprotein quality and composition
- Inflammation and oxidative stress
- Metabolic health and insulin resistance
- Thyroid dysfunction and hormones
- Vitamin and nutrient deficiencies
- GI, liver and gallbladder function
- Sleep, stress, alcohol, and drug use
- Exercise patterns, including overtraining
- Overall clinical context
Cholesterol in Context
We help you understand what your cholesterol numbers actually mean, not simply whether they are "high" or "low."
More importantly, we help answer:
- Is this cholesterol pattern actually contributing to cardiovascular risk?
- What factors may be driving it, and how can they be addressed?
By evaluating the full picture of your cardiovascular health, we help create a personalized strategy that goes beyond simply lowering a number.
Through our education platform, we also teach you how to better understand cholesterol, make evidence-based lifestyle changes to address it, ask informed questions, advocate for yourself, and partner more effectively with your healthcare team.
A deeper understanding helps prevent undertreatment, overtreatment, and misdirected treatment.
A Healthcare System That Doesn't Have Time for You
Most medical visits last only 10–30 minutes and happen only once or twice per year. No clinician can fully evaluate dozens of cardiovascular risk factors, connect the dots, and create a personalized plan in that limited time.
Even when working with multiple providers — a PCP, cardiologist, nutritionist, or therapist — care often remains fragmented, with no one person synthesizing the complete picture of your health.
Patients are often left to:
- Research answers on their own between visits
- Advocate for additional testing
- Coordinate fragmented care
- Make decisions without enough time to understand all their options

Incomplete Conversations
Doctors can only counsel patients based on what their training taught them and with the limited time available during an office visit. As a result, important conversations about the evidence, benefits, risks, limitations, and uncertainties behind medical recommendations often never happen.
Many patients feel:
- Medication side effects are minimized or dismissed
- Questions are discouraged rather than explored
- Treatment limitations and alternatives are not fully discussed
- They are uncomfortable asking questions or challenging recommendations
This is especially true for complex and sometimes controversial topics such as cholesterol management and statin therapy, where patients often want to better understand:
- Who is most likely to benefit
- The magnitude and limitations of benefit
- Potential risks, side effects, and trade-offs
Medicine is constantly evolving. Evidence changes, and uncertainty is a normal part of science. Yet many patients feel healthcare leaves little room for curiosity, discussion, or critical evaluation of the evidence.
Many patients tell us they feel like they are frustrating or even angering their doctor if they ask questions or decline or defer starting the recommended medications. Too often, the relationship feels less like a partnership and more like comply or goodbye.
Full Transparency and Openness
At 3 Billion Beats, we believe the best healthcare is built on transparency, curiosity, humility, and partnership.
We encourage open-minded, evidence-based conversations about heart disease that are often missing from healthcare. No question is off limits. There are no unwelcomed questions or taboo topics. We examine the science from multiple perspectives, discuss where the evidence is strong, where it is evolving, and where uncertainty remains.
Our goal is not to tell you what to think. It is to help you understand:
- Why a recommendation is being made
- Its benefits, risks, and limitations
- What other factors may be contributing to your health
- All reasonable options for prevention, testing, and treatment and how they compare
We believe healthcare works best when patients and clinicians evaluate the evidence together and make informed decisions based on the complete picture — not just a population level guideline, lab value, or prescription.
Our recommendations are guided by the scientific evidence of doing what works and what feels best for you — not by politics, financial incentives, or the constraints of a rushed office visit.
